Before you Start

During the completion of this form you will be required to upload proof of documents. Please have all documents ready before completing the form. Documents include:

  • Teacher Reference Number (can be found on your QTS certificate)
  • DBS Certificate
  • Update Service Number (if applicable)
  • Head and shoulders picture of yourself
  • Copy of safeguarding training certificate (less than 12 months old)
  • Unique Taxpayer Reference Number
  • Contact details of 2 referee’s
  • Proof of identity (Copy of Passport or Birth Certificate and National Insurance Number)
